Parkway West Band Important Forms

The Administration requires that all Band and Color Guard students have a physical examination, and that a standard health form be kept on file with the school nurse. Any physical exam performed after February 2010 will be acceptable. Also required is a combined health and permission slip that must be notarized. This form is maintained in the band office and taken along on all trips.

The required forms are as follows:

1.	Parkway School District Physical Examination form - Each student needs to have a physical exam performed by a physician as of February 2005 or later. Requires parent/guardian signature, signature of student, and the physician must sign this form. Mail this form to the school nurse (see below). 2.	Permission and Health form - This form must be NOTARIZED. It allows the band director to receive emergency medical treatment for your child if necessary. This form always accompanies the band where ever it goes. Mail this form to the Director (see below). Click here to view and print this form. (requires Adobe Acrobat 4.0 or higher).
3.	Student Emergency Card - This form also travels with the band and provides emergency contact numbers for the Band Director should it be necessary to contact you. 4.	Health Update form - This form provides an update of your student's health as well as information about the routine medications that they take. Mail this form to the Director (see below). Click here to view and print this form. (requires Adobe Acrobat 4.0 or higher).
